Hastanelerde İlaçların ve Tıbbi Malzemelerin Stok Kontrolü

Çalışmanın Amacı: Hastanelerde stok kontrol sürecinin etkinliği, hem hasta bakımında iş süreçlerinin aksamaması için hayati rol oynamakta hem de maliyetlerin kontrol altında tutulmasını sağlamaktadır. Bu çalışmanın amacı hastanelerde ilaç ve tıbbi malzeme stoklarının etkin şekilde kontrolünde kullanılan ABC-VED matrisi yöntemini değerlendirmektir. Yöntem: Çalışma hazırlanırken ikincil verilerden yararlanılmıştır. Bu kapsamda ilaç ve tıbbi malzeme stoklarının etkin şekilde kontrolünde tek bir stok yönetim sisteminin uygulanamayacağını ortaya koymaya yönelik literatür taraması gerçekleştirilmiştir. Konu ile ilgili yayınlanan kitaplar, süreli yayınlar, bildiriler ve raporlar taranarak mevcut durumun ifade edilmesi yönünde tanımlayıcı bir araştırma yapılmıştır. Bulgular: ABC sınıflandırmasının esas amacı, doğrudan yıllık en fazla harcamalara neden olan stok kalemlerine odaklanmaktır. A sınıfı stok düzeyleri azaltılabilirse, stok yatırımlarında önemli düşüşler sağlanabilmektedir. Bu yöntemin temel kısıtı, fiyat ve ürünlerin kullanım yüzdesine bağlı olmasıdır. Sermaye yatırımı ve tüketimi düşük ama hayat kurtarıcı bir stok kaleminin önemini tam olarak değerlendirememektedir. Bu nedenle hastanelerde ilaç ve tıbbi malzemelerin stok kontrolünde ABC yöntemi kullanılırken aynı zamanda stok kaleminin kritikliğini de göz önünde bulundurmak için VED kritiklik analizi de yapılmaktadır. “V” grubu, hastanelerde her an bulundurulması gereken hayati stok kalemleridir. Bu ilaç ve tıbbi malzemelerin kısa bir süre bile mevcut olmaması hasta bakım hizmetlerini ciddi şekilde aksatmakta ve hastanenin işleyişi olumsuz etkilenmektedir. ABC-VED matrisi üç kategoriye ayrılmaktadır. İlk kategori AV, AE, AD, BV ve CV gruplarına dahil olan stok kalemlerinden oluşmaktadır. Bunlar yüksek, orta ve düşük parasal değere sahip olmakla birlikte her zaman mevcut olması gereken hayat kurtaran ve kesintisiz sağlık hizmeti için stokta bulunması gereken kalemlerdir. İkinci kategori BE, CE ve BD gruplarına dahil olan stok kalemlerinden oluşmaktadır. Orta veya düşük parasal değere sahip, hastalıklar için önemli ancak hastanede bulunması gereken hayati ilaçlara kıyasla daha az şiddetli olan ilaç ve malzemelerdir. Eksikliği kısa bir süre için tolere edilebilmekte veya hastanede alternatifi bulunabilmektedir. Üçüncü kategori CD grubuna dahil olan stok kalemlerinden oluşmaktadır. Bunlar, parasal değeri düşük olan, eksikliği hastaların sağlığına zarar vermeyecek malzemeler olarak sınıflandırılmaktadır. Sonuç ve Öneriler: Etkin stok kontrolü sağlık malzemesinin zamanında tedarik edilmesini, sağlık hizmetlerinin devamlılığını, atık bertaraf ihtiyacının azalmasını ve maliyetlerin kontrol altında tutulmasını sağlamaktadır. Malzemeler önem derecesine, yıllık tüketim miktarına, her bir malzemenin ortalama birim fiyatına ve son kullanma tarihine göre stok sisteminde izlenmektedir. Stoklarda kayıp yaşanmaması için tedarik sürecinin her aşamasında taşıma ve depolama süreçlerinin izlenmesi, son kullanma tarihi gelen ilaç ve tıbbi malzemelerin kullanılması için uygun stok rotasyonunun sağlanması gerekmektedir. İlaç ve tıbbi malzemelerin emniyet stok seviyelerinin belirlenmesi için hastanenin coğrafi konumu, hedeflenen büyüme öngörüsü, beklenen hasta sayısı, kliniklerden alınacak periyodik malzeme kullanım raporları, bütçe ve nakit imkânları gibi faktörlerin dikkate alınması gerekmektedir. Literatürde ABC-VED matrisi kullanılarak hastane eczaneleri, acil servisler, özel eczaneler, medikal malzeme tedarikçileri, üreticiler gibi sağlık sektörü ile ilgili farklı birim ve işletmelere yönelik araştırmalar bulunmaktadır. Gelecek araştırmalarda bu analizlerle elde edilen sonuçlar karşısında talep belirsizliği yaşanan ilaç ve malzemelerin tedarik yöntemlerinin ve iş yükleri üzerine etkilerinin açıklandığı çalışmalar yapılabilir.

Inventory Control of Drugs and Medical Supplies in Hospitals

Introduction: The efficiency of the inventory control process in hospitals plays a vital role in both keeping the business processes in patient care and keeping the costs under control. The aim of this study is to evaluate the ABC-VED matrix method used in the effective control of drugs and medical supplies in hospitals. Method: Secondary data were used for the study. In this context, a literature review was conducted to reveal that a single stock management system cannot be applied in the effective control of drug and medical supplies stocks. A descriptive review was conducted to convey the current situation by scanning the books, periodicals, papers and reports published on the subject. Results: The main purpose of ABC classification is to focus directly on the stock items that cause the highest annual expenditures. If Class A stock levels can be reduced, significant reductions can be achieved in stock investments. The main limitation of this method is that it depends on the price and the percentage of use of the products. Capital investment and consumption are low but cannot fully evaluate the importance of a life-saving stock item. For this reason, while ABC method is used in stock control of drugs and medical supplies in hospitals, VED criticality analysis is also performed to take into account the criticality of the stock item. The "V" group are vital stock items that should be kept in hospitals at all times. The absence of these drugs and medical supplies, even for a short time, seriously disrupts patient care services and adversely affects the functioning of the hospital. The ABC-VED matrix is divided into three categories. The first category consists of stock items included in AV, AE, AD, BV and CV groups. These are items that must be in stock for life-saving and uninterrupted health care, which have high, medium or low monetary value, but should always be available. The second category consists of stock items included in BE, CE and BD groups. They are drugs and supplies of medium or low monetary value, important for diseases, but less severe compared to vital drugs that need to be in hospital. Its deficiency can be tolerated for a short time or an alternative can be found in the hospital. The third category consists of stock items included in the CD group. These are classified as materials with low monetary value and whose deficiency will not harm the health of patients. Conclusion and Recommendations: Effective inventory control ensures timely supply of healthcare supplies, continuity of healthcare services, reduction of waste disposal need and keeping costs under control. Materials are monitored in the inventory system according to their importance, annual consumption amount, average unit price of each material and expiry date. In order to avoid loss in stocks, it is necessary to monitor the transportation and storage processes at every stage of the supply process, and to ensure appropriate stock rotation for the use of drugs and medical supplies with expiration dates. In order to determine the safety stock levels of drugs and medical supplies, factors such as the geographical location of the hospital, targeted growth forecast, expected number of patients, periodic material usage reports to be obtained from clinics, budget and cash facilities should be taken into account. In the literature, there are studies on different units and businesses related to the health sector such as hospital pharmacies, emergency services, private pharmacies, medical supplies suppliers, manufacturers using the ABC-VED matrix. In future studies, studies can be conducted to explain the effects of the supply methods and workloads of drugs and materials with uncertain demand in the face of the results obtained with these analyzes.
